Diabetes and its comorbidities are highly preventable and treatable. Yet there are multiple challenges in translating evidence to practice which involve capacity building, behavioral changes and strengthening of our health care systems in order to bring out the best of our clinical expertise and technology. In this annual forum, we have invited thought leaders to share their experiences and expertise in pursuit of these complex goals. Through these deliberations and interactions, we hope to build a network of care professionals and experts who share similar vision to innovate and transform health promotion and disease prevention.

Asia Diabetes Foundation (ADF) is a charitable organisation governed by the Chinese University of Hong Kong Foundation. Since its establishment in 2007, the Foundation has initiated and implemented various scientific research in Hong Kong and Asia regions to collect and translate clinical evidence into prevention and control strategies for diabetes and other chronic diseases. ADF is dedicated to the advocacy and support people with diabetes through raising diabetes awareness and providing healthcare support.

Association of Hong Kong Diabetes Nurses (AHKDN) is dedicated to driving the professional development and practice of its members, advancement of the diabetes management standard and thus improvement of living for people affected by diabetes in Hong Kong.

Asia Primary Care Diabetes Society (APCDS) aims at facilitating all medical practitioners, allies health professionals and care providers to update their medical knowledge, share clinical experience and give mutual support in diabetes and chronic diseases management in primary care.

Diabetologists & Endocrinologists Alliance (DEA) is an alliance of diabetologists & endocrinologists in private practice. We aim to promote public awareness and provide public education in Diabetology & Endocrinology. We encourage continuous medical education among members to upkeep the standard of practice in Diabetology & Endocrinology. We co-ordinate and co-operate with other associations interested in diabetology & Endocrinology. Our members extend to other physicians and nurses.
